IVA WELL WAIVER
I consent that I am of sound mind and in good physical health, and do not suffer from any conditions that would prevent me from participating in any IVA WELL activities. Such activities may include, but are not limited to talk therapy, aromatherapy, herbalism, sound therapy, reiki, light massage, tapping, meditation, and movement.
I understand that my participation in these sessions held by Julia Gelfgatt and her associates is of my own free will and strictly for entertainment purposes. I understand that any medical concerns should be directed under the care of a medical professional and that IVA WELL sessions are not meant to be a substitute for medical or psychological diagnosis and treatment. I understand that it is my responsibility to consult with a medical professional before taking any herbal remedies recommended by Julia Gelfgatt, her colleagues, and or employees, and to alert Julia Gelfgatt and her associates of any allergies or sensitivities to essential oils and plants prior to the session. Any advise given is ultimately determined by my own free will and I release any liability to sue Julia Gelfgatt, IVA WELL, and it’s associates or contractors.
My participation in all IVA WELL activities is confirmation that I am fully responsible for any and all, risks, injuries and damages, known or unknown without limitation.
I hereby release my image should photos be taken in group sessions that may later be used in social media, websites, or for any press purposes.
By signing this release, responding to this email, showing up to any IVA WELL session or workshop with Julia Gelfgatt, or her associates, employees, or contractors, I acknowledge that I have read and completely understand the terms of the release, that I am legally of sound mind, and voluntarily agree to the terms and conditions stated above.
